Preparing for Your Blepharoplasty

5 Reasons Why You Are the Perfect Candidate for Lipo | NYCLipo techniques have been perfected over the last few decades, and there are more options available than ever before. Most men and women 18 and over are good candidates for liposuction, but there are certain aspects of your health and lifestyle that could make you the ideal candidate for lipo including:

1. You are at or near your goal weight, but have stubborn areas of unwanted fat such as your flanks (love handles), abdomen and thighs. Liposuction isn’t a solution to obesity. Removing more than 6-10 pounds of fat in one procedure significantly increases your risk of complications.

2. You live a healthy lifestyle full of exercise and are otherwise in good health. The best candidates for lipo already follow a healthy diet and exercise regimen and don’t have pre-existing conditions that could cause complications such as heart disease, lung disease, diabetes and poor blood circulation.

How to Get Rid of Excess Fat & Cellulite | NYC, ManhattanMany women struggle with cellulite, and this annoying, visible rippling can worsen with weight gain. Patient Goals

Surgical Training

Is PRK or LASIK Better for Vision Correction? | Manhattan, NYCPRK and LASIK laser eye surgeries utilize different techniques, but both deliver excellent results. The question is not which procedure is better, but rather which one is better for your eyes? Both laser eye surgeries reshape the cornea to correct your vision, but they differ when it comes to your corneal surface. LASIK creates a flap using the surface layer (epithelial layer) of your cornea, while PRK removes this layer through ablation.

Our board-certified ophthalmologist can help you decide which form of vision correction will give you the results you're looking for, but these questions can help you get a better idea of which one is best for your eyes:

  1. Has your vision been stable for one or more years? PRK and LASIK can both be performed on someone who has had a stable prescription for one to two years and is at least 18 years old. Some ophthalmologists suggest PRK is better for those over the age of 40.
  2. Do you have thin corneas? Our laser eye surgeon may recommend PRK over LASIK if you have thin corneas because PRK removes the surface layer of your corneas instead of creating a flap like LASIK. This allows your eyes to regenerate the epithelial layers.
  3. How much time do you have for recovery? PRK can take several days or weeks to recover from as your eyes have to grow back the surface layers removed during the procedure. LASIK recovery requires as little as one day.
  4. Do you have dry eyes? PRK is usually the best choice if you have dry eyes because it doesn't interfere with tear production or sever nerves to create a flap.
  5. Do you have an eye disease? Laser eye surgery is not a great option for you if you have cataracts, glaucoma or other eye diseases and conditions.

Which Has Better Results: PRK or LASIK?

The results from PRK and LASIK are virtually identical, but sometimes PRK offers a better outcome when it comes to achieving perfect vision. Cataract Awareness Month | 7 Foods to Prevent CataractsEye care becomes more and more important as you get older. Age-related cataracts are one of the leading causes of vision loss and blindness throughout the world. Studies suggest a healthy diet can prevent or slow down the oxidation process associated with cataracts.

The exact cause behind cataracts is unknown, but oxidation plays a major role. Oxidative stress is caused by an imbalance between free radicals (which could be caused by poor diet) and natural antioxidants found in your body. This oxidation process causes the breakdown of enzymes and proteins in your eye, which in turn causes your eye’s natural lens to become cloudy, creating a cataract.

The only effective treatment for this eye disease is cataract surgery, but a diet rich in these foods could lower your risk of developing cataracts:

  1. Spinach or Kale. Green leafy foods are full of lutein — a vital nutrient for healthy eyes. If you already have cataracts, make sure your diet contains a lot of spinach or kale.
  2. Cold-Water Fish or Walnuts. Fish such as salmon, herring or tuna are full of omega-3 fatty acids. This nutrient is heavily concentrated in your retina, and a diet full of fish can keep the back of your eye healthy. For vegetarians or vegans, nuts such as walnuts or pistachios are also great sources of omega-3 fatty acids.
  3. Berries. All forms of berries including grapes are fantastic for your overall health. Berries can lower your blood pressure and decreases inflammation, which can both be detrimental to your eyes. Blueberries and blackberries are especially great for your eyes because they can stop the arteries that supply oxygen to your retina from becoming blocked.
  4. Orange-colored foods. Zeaxanthin is another nutrient naturally found in the back of your eye. Orange foods, specifically orange bell peppers, have high zeaxanthin and lutein levels as well as vitamins C and A. Vitamin A has been proven to help night vision.
  5. Tea. A recent study found that green or black tea can prevent cataracts from forming in animals. Tea contains high levels of antioxidants along with vitamins A, C and E.
  6. Eggs. An egg per day could keep cataracts at bay. Eggs contain nearly all of the nutrients and vitamins necessary for healthy eyes including lutein, zeaxanthin and omega-3 fatty acids. While diabetics should first consult their doctors about egg consumption, a majority of those at risk for cataracts can safely eat eggs daily.
  7. Avocados. This fruit is one of the most nutritious foods you can eat. Avocados promote your overall health, making them excellent sources of nutrition for your eyes. They are filled with vitamins and nutrients such as:
    • Lutein
    • Beta-carotene
    • Vitamin C
    • Vitamin B6
    • Vitamin E

What’s the Difference Between PRK and LASIK? | ManhattanPRK and LASIK are different methods of laser eye surgery. Both can correct vision problems such as nearsightedness, farsightedness and astigmatism, but each one has unique benefits. The right surgery for you depends on your corneas, and the best way to find out if you qualify is by scheduling a consultation with our board-certified ophthalmologist.

PRK (photorefractive keratectomy) was the precursor to LASIK and is still widely used across the country including by Dr. Basil Pakeman. The main difference between these forms of vision surgery is how they are performed.

PRK uses a laser to reshape your cornea and correct your eyesight after removing the surface layer called the epithelial layer. PRK can contour more of your cornea because the laser has access to your entire eye. LASIK creates a flap using the epithelial layer then reshapes your cornea. Once the procedure is done, the flap is folded back over your eye.

PRK and LASIK both take minutes to complete in our Manhattan office, but healing time varies. Because PRK removes the entire surface layer, it can take several days for your body to naturally form a new one. Your full results can take up to a couple weeks to stabilize. LASIK also takes a few days to recover, but has a quicker healing rate.

Good candidates for PRK must also avoid medications including Accutane, Cordorone, Imitrex, and steroids for a period of time leading up to surgery. If you take one of these medications it is important to work with both your prescribing physician and Dr. Pakeman to determine the safest and most effective way to discontinue use.

Because the epithelium is completely removed during PRK, full recovery can take up to one week. Most people are able to return to work within 24 hours, but each case is different. However, a meta-analysis of several studies assessing these risks has found highly inconsistent and inconclusive results.

Because results were mixed and no study pointed to serious risks, the study’s authors suggest that the risks of increased depression symptoms and discontinuation syndrome far outweigh the risks of bleeding, and suggest patients do not discontinue antidepressants prior to their procedure.

Common Liposuction TermsAs you research liposuction, you may find it helpful to know the definitions of common terms:

  • Liposuction: A cosmetic surgical procedure that removes excess fat from various areas of the body.

  • Avoiding permanent pigmentation – Darker pigments can act like a tattoo and become permanent along the incision line as your skin is healing. Even if the incision has sealed, the tissue may absorb the pigments.

Am I Too Young For Eyelid Surgery?According to Cleveland Clinic, eyelid surgery is a common procedure among people age 30 and older. Contact Lenses linked to Saggy EyelidsA new study says wearing contact lenses increases the odds you'll experience droopy upper eyelids, a cosmetic and sometimes visual problem corrected with blepharoplasty (eyelid surgery). The study was published in Aesthetic Surgery Journal, which is the official publication of the American Society for Aesthetic Plastic Surgery.

Apparently, researchers found increased risk of droopy eyelids (called "ptosis") was associated with the use of hard lenses and soft lenses. The study did find that ptosis was more severe for people who wore hard contact lenses.

The study compared the eyelid changes of 96 pairs of identical twins. Because identical twins have the same facial characteristics, marked changes are typically contributed to environmental – instead of genetic – factors. The researchers found "wearing contact lenses was the only external factor that was linked" to upper eyelid drooping. Other possible contributors – such as sun exposure, smoking, alcohol use and stress – were ruled as not having a "statistically significant impact" on upper eyelid drooping.

The findings seem to indicate that the constant stretching of your eyelids when you insert and remove your contact lenses is what is causing drooping of the eyelid skin. Manhattan Eyelid SurgeryIn short, yes, eyelid surgery does leave scars, but these scars are virtually impossible to see. Please see our before-and-after gallery to see real-life results of some of Dr. Basil Pakeman's eyelid surgery patients. The location of the scars depends on where the incisions are made to perform the blepharoplasty procedure:

Upper eyelids: Incisions are typically made in the natural crease of your upper eyelid. Typically the resulting scars are significantly concealed within the fold of the skin.

Lower eyelids: Incisions are made along the lower lash line where they are well hidden. If you are having lower eyelid surgery to remove bags of excess fat beneath your eyes with a "transconjunctival" approach, then an incision will be made on the inside of your lower eyelid where no one can see it. The transconjunctival approach removes fat only, and cannot address sagging skin.

After Dr. Pakeman completes the blepharoplasty procedure, he will suture the incisions closed with very fine stitches to minimize scarring. Once your scars heal, you can enjoy a more youthful, alert facial appearance with well concealed scars.
